The following forms are required prior to beginning certification training.
Medical Statement (click to download in pdf format)
This form needs to be filled out completely answering YES or NO to the pre-existing medical conditions listed. An indication of YES for any question of the medical statement will require the student download and comply with the Physician Guide.
The student must arrive with the medical statement (and physician's statement if required). These are as important as VISAS in a passport.
